What it means

Said of something that has just gone wrong, usually by whoever is responsible.

Where it comes from

Military planners have a saying that no plan survives contact with the enemy, which is normally attributed to Helmuth von Moltke. He wrote it in the 1870s, and he was not being reassuring.
